Add a check on resting for Clerics (and similar divine casters) to see which spells that are "praying" for.
For each deity, there should be a list of available spells that fit the deity's "theme" and/or domains. If the cleric is praying for a spell not on the list, there should be a chance that the spell is either not granted or is replaced by a more deity-approved spell.
In my mind, this would be done as a list of spells with two numbers following each spell; the first is the percentage chance that the spell will be granted, the second is the percentage chance that no spell will be granted at all.
E.g., a "warlike" deity may think that Sanctuary isn't a great idea, and so may decide not to grant it:
Sanctuary 60 25
If the character requests the "Sanctuary" spell, there's a 60% chance it will be granted; if it isn't granted, there's a 25% chance of getting nothing in that slot, and 75% chance of getting a random spell off of the deity's favored list.

Should be easy enough under the original idea... if it's not on the deity's list, treat it as 100% granted.

Also sounds like a good idea.
One other thing I forgot to put in the original post... if you've already established a % chance for successfully getting a spell granted, then it shouldn't be too hard to add in modifiers. I.e., if you take actions or deviate in alignment away from your deity's dogma, you might get an across-the-board penalty to your "approval" chance -- exceptional faith/adherence could have a bonus.
It IS a nice idea to be sure as it is , unlikely to say the least, for a guy like Gruumsh to give out a sanctuary or bless, but it would be hell to code it in. Anyway if someone mods it in I think it will be great!

An easier version of this might be to have a fairly short list of "disliked" spells for each deity. Any requested spell not of the "bad" list will be granted automatically. Any requested spell that is on that deity's "bad" list would have a chance of not being granted like you have described. I would also specify that there is only one check for a particular spell even if you request it more than one time. This way, you can't request 2 Sanctuary spells (to use your example) and hope to improve your odds of getting at least one.
I think that this sort of idea is behing clerics having domains.
